The Employee Portal for Seeley Union School District employees is now available! The online portal allows staff to view personal information such as HR/Payroll information: contact information, tax statements, payroll check-stubs, vacation/sick leave balances, and information on your health insurance. Employment Verification I-9 Form

Form I-9 is used for verifying the identity and employment authorization of individuals hired for employment in the United States. All U.S. employers must ensure proper completion of Form I-9 for each individual they hire for employment in the United States.

2020 Form W-4

Complete this form so that your employer can withhold the correct federal income tax from your pay. Consider completing a new Form W-4 each year and when your personal or financial situation changes.

California DE-4

This certificate, DE 4, is for California Personal Income Tax (PIT) withholding purposes only. The DE 4 is used to compute the amount of taxes to be withheld from your wages, by your employer, to accurately reflect your state tax withholding obligation.

Employee Timesheet

Fillable Timesheet on PDF. To submit timesheet, please complete all required fields: *Name, Position, Date, Time In/Out, # of Hours, Purpose (notes), Total, Sign and Date.

You must "SAVE AS" and email to fornelas@seeleyusd.org. If you do not save as and send updated file, it will be sent blank. 

Automatic Payroll Deposit Form

For Direct Deposit, complete the form attached to create, change or cancel payroll options. 

Employee Information Change Request Form

Employees with changes such as mailing addresses, name changes, contact updates should complete the attached form and submit to the payroll office.
